How to Submit a Group Employee Benefit Claim

All of the necessary medical forms are located here to submit a Group Employee Benefit claim. The forms have contact information and helpful tips that are specific to each type of claim.

To ensure that your claim is processed without delay, ensure that you include your: 

  • Name
  • Date of birth
  • Policy/Plan number
  • Employee Identification number 

Remember to sign and date the form and submit it to the address on the Claim Form. KEEP A COPY FOR YOUR RECORDS
