There are two ways to submit a MEARIE Group Employee Benefit Claim.

To submit a paper-based Group Employee Benefit claim, please click to access all of the necessary medical forms. These forms have contact information and helpful tips specific to each type of claim. Remember to sign and date the form and submit it to the address on the Claim Form. KEEP A COPY FOR YOUR RECORDS.
